Abstract
A main objective of the D-CARE project is to improve the skills and develop, validate and scale up smart solutions that will strengthen and integrate the regional social and healthcare systems in 9 Danube regions. 2) Deploying Smart Care Pilots to explore, select, optimize, test and validate innovative smart care services for older adults with chronic diseases or cognitive impairments;
3) establishment of Transnational Learning Policy Center that should ensure the generation of know-how and the dissemination of results and knowledge to policy-makers.
